Prepare for Your Immigration Physical
To make the immigration medical exam appointment run smoothly, you’ll want to bring:
- Report of Medical Examination and Vaccination Record (form I-693)
- Photo Identification (Photo ID) (government-issued) (valid passport or driver’s license is fine)
- Immunization record
- Medical insurance card
- Payment for the USCIS Doctor visit
You should fill out the I-693 form (adjustment of status) to the extent you are able and bring it with you to the exam.
